Male breast enlargement is a condition that bothers many of our gynecomastia New York City & Long Island patients. It is a condition where the breast tissue is enlarged, making the breast appear fuller than the typical male breast.
Obesity can also cause an enlargement in the fat tissue in the male breasts, but this condition is called pseudogynecomastia. “Pseudo” or false gynecomastia may look the same but it is not considered a true medical condition.
True gynecomastia, however, is the enlargement of the fatty and or glandular tissue in the male breasts that cannot be reduced or eliminated through diet and exercise. It is usually caused by a hormonal imbalance where the male androgen hormones are at a lower level in comparison to the female estrogen hormones within the body.
Causes of Gynecomastia
When this develops during childhood or puberty, it often resolves itself within two to three years and hormone levels return to their proper levels. Adult men over the age of 50 can also develop male breast enlargement due to an imbalance of hormones. However, other medical conditions, treatments and medications can also cause the development of enlarged breasts.
These include conditions like malnutrition, chronic kidney failure, hyperthyroidism and drugs such as some anti-ulcer drugs and calcium channel blockers. Drug and alcohol abuse can also lead to gynecomastia.
Mild, Moderate, and Severe Gynecomastia
Gynecomastia can be minor, moderate or severe. For minor cases we see in our gynecomastia New York City & Long Island practices, there is no real evidence of excess skin. The areola area is thrust forward by a little bit of breast tissue. In these cases, the nipple appears pointy. For minor cases, male breast reduction surgery consists of liposuction alone to correct the condition. This is a relatively short procedure that is always conducted under local anesthesia and light sedation.
Two incisions are made in each breast. Each incision is very small, generally between two to four millimeters and is made in an inconspicuous location. In moderate cases of gynecomastia, there is still no excess skin but a larger amount of glandular tissue protruding from the areola area. Rather than pointy nipples, the breast appears slightly enlarged at the areola. Moderate gynecomastia is generally treated with laser-assisted liposuction and the surgical removal of the excess glandular tissue.
The incisions for the liposuction are the same as for minor gynecomastia followed by a small incision made at the bottom of the areola skin on each breast to hide the scar easily. This procedure is carried out using local anesthetic and light sedation. The scars from this procedure are generally unnoticeable once the area heals.
In cases of major gynecomastia, the breast area appears enlarged and there is excess skin. Laser liposuction is performed to remove the superfluous breast tissue and fat. Incisions are then made to remove the excess skin and glandular tissue. The Advanced Cosmetic Surgery Team will make the incisions in the most inconspicuous locations, but one risk is that scars may be evident once the area has healed. This is an uncommon result. Scars will fade over time to nearly invisible as the skin heals itself. There will be some bruising and swelling postoperatively with each type of procedure, but even in cases of major gynecomastia, the full results of the surgery will be evident within a few months.
